Visit Form

Vet Form

The Good Dog Vet Form must be updated annually for Good Dogs to maintain their active status. When needed, please download the Vet Form, have your vet fill it out, and email to the Good Dog office (or upload it with your other documents via the Recertification form).

Incident Report Form

Please fill out this incident report form if anything irregular, upsetting or against Good Dog and/or facility policy happens on an official Good Dog therapy dog visit. This includes issues with other volunteers, dogs, facility staff or clients. Please send it our office immediately to protect yourself and the organization. You can email it to:

Annual Re-certification

Re-certification is important in order to ensure compliance with state law and Good Dog policies, and enable your continued coverage under The Good Dog Foundation liability insurance while on visits. You may not visit any facilities as a Good Dog team past the certification expiration date on your ID badge.

Steps for Re-certification:

  1.      Pass an in-person re-certification evaluation with a Good Dog trainer*
  2.      Submit (1) Volunteer Agreement (2) Vet Form (3) Re-certification fee

*Expectations for the Re-certification Evaluation:

Handlers should treat the appointment like a therapy dog visit. Dogs should be leashed at all times and not be permitted to interact with other dogs in the room. The dogs will be expected to demonstrate Sit, Down, Stay, Leave It, Come, polite loose leash walking skills, and also greet politely without jumping. All of these tasks should be done without the use of treats. If there is more than one handler on your team,
all handlers should attend the evaluation.

Once your re-certification process is complete, you will receive a new Good Dog ID badge with an updated expiration date. The Good Dog office will mail badges within two weeks of the completion of all re-certification steps.

All teams must re-certify each year to continue visiting.  Good Dog will send you re-certification email reminders with more details two months and one month prior to your expiration date.  Make sure to check your spam/junk folders in case our emails end up there!  If two years pass after your expiration date, and you decide to resume visiting, you will have to re-take therapy classes.
